• Contact a friend from a fellow agency and introduce DSL as their possible software. This prospect agency should request for a demo at bit.ly/DSLDemoRequest. Make sure this prospect puts in YOUR NAME, EMAIL AND CONTACT NUMBER in their submission for you to be considered in our Spread the Word Program. Note that the prospect must not have any pending negotiations with DSL in the last 3 months.
• Data Soft Logic’s Sales Executive will validate your referral with the prospect. Once validated, you will be asked to complete the Referral Form below.
• Monitor your referral for their scheduled demo.
• The last step will be your referral signing their Software Subscription Agreement (SSA). Only when the prospect submits these to DSL within 90 days of the date of the referral, and pays the set up fee in full, will you receive the first half of the reward ($500)
• After your referral starts their regular (professional, premium, or corporate) subscription plan and stays in good financial standing for 2 consecutive months, you will receive the remaining half of your reward ($500).

Data Soft Logic agrees to appropriately and lawfully respond to any and all requests regarding email and fax communications. 
• If the referral is a Startup agency (awaiting PTAN issuance), the referral amount will be awarded once the agency is Medicare-certified with two months of Data Soft Logic service paid in full. The IRS considers this taxable compensation. Accordingly, Data Soft Logic requires each recipient to complete a W-9 so that we may provide this information to the IRS via a 1099-MISC. Should Data Soft Logic submit this information to the IRS, you will also receive a 1099-MISC mailed to the address on the W-9. 
• Data Soft Logic reserves the right to amend, adjust, change, or discontinue any and all terms of this program at its sole discretion without prior notice. Data Soft Logic retains sole authority to interpret all aspects of the referral program including, but not limited to, criteria, eligibility, amount, and whether payments will be made.
