An Interdisciplinary Team or Group, more commonly referred to as IDT or IDG, is a collaborative group of professionals from various disciplines who work together to attend to the holistic needs of hospice patients and their families and friends. To develop and review the Plan of Care (POC), the IDT meets at least once every 15 days. These bi-weekly meetings ensure that every IDT member, whether core or non-core, provides care services that are consistent with the hospice philosophy and the patient’s POC.
